Next hack I will be developing will be a feedback / support contact form for plugins within b2evo.

So if the plugin has the hack added, then there will be a tools page called 'Plugin Author Contact' (can you think of a better title?).

Inside there will be:
A combobox called "Plugin" that contains a list of all supported plugins.
A combobox called "Reason" that contains "General", "Feedback", "Support"
A textbox for the title of the email
A textarea for the content of the area
And if support is the reason a combobox will appear (which will be checked by default) called "Include Debug Information", with a note '(Contains information helpful to the developer to debug your problem, like your blog url, plugin version, plugin settings, and any debug logs)'.

And upon plugin install a message will appear saying "To provide feedback or get support, you can use a form in the "tools" section"

Any ideas / suggestions?
