My b2evolution Version: 1.9.x
I have a private blog where only registered members can view and post comments. I also have the comments set up so that they have to be approved before they are posted to the blog.
The problem is that anyone that has subscribed to receive email notifications on comments will still get an email of a comment BEFORE I approve it!
Is this a bug? How can I prevent users from receiving emails with comments before I approve the comment?

That's a "bug" or rather a not-implemented feature. I'll add TODOs at the appropriate places, but you should not expect this to be fixed before 2.0.
