Just a guess, try to open the image in any editor and save it with different parameters.

Tried that as well. I saved the image in both PNG and JPG format and neither work. Permissions on the server are right, but the image will not appear in IE8 at all.

For kicks :
1) change your alt tag to something that doesn't include [] or the word error
2) change your css for pageHeader to remove the broken background url

i set the header tag and it still shows what looks to be the website title tag for the image.
i also removed the broken image link from the css file and still the picture does not show up.
is there a different plugin for an image title widget?

Just hard-code the image tag in skins/[SKIN NAME]/_body_header.inc.php
