Sure you can. It's often easiest to find a currently released skin that more or less has the layout you're looking for (# of columns, fixed-width, or whatever) and then tweak out the css and images to get it the way you want for your own skin.

This way you're sure to have all the necessary skin files and php sections to make your skin function well.
