Wouldn't that just be a normal, fresh install on your new host?
FWIW ... I'd copy the blog folder over, then make a backup of my old DB (at the old host) and restore it to a newly created DB (same name, password) at the new host. Your blog should then be intact and usable at your new host, then.
(If you're moving your domain "yoursite.com" to your new server, you'll have that to do, as well.)
