One problem may be that it appears as though the user is commenting on that specific comment as in a heirachy of comments/ i.e. starting another thread, rather than add one to the list.

It is likely a skin issue. Each skin decides what and where to show buttons etc. There may be a widget for extra buttons etc.

You would have to say which version b2e and which skin to get more to the point.

if you can code a bit you can add a button to the skin you are using.
