My opinion is probably biased because I run the antispam database, but I use the central blacklist and a fairly robust .htaccess file to block some keywords. I think the .htaccess file takes care of most stuff for me, and sometimes (this is hard to understand) I can't ban a spammer because my .htaccess blocks me from banning them. I can't figure out how they get in if I can't block them, but they do and I can't. Rare, but strange.
Thanks a lot EdB, OK, I'll will use the Blacklist. I don't use comments very much at all, maybe on a particular post once in a while where I want t reply.

For comment spam I use my turing test plugin.
For message form spam I use a hack ¥åßßå wrote that runs the messages through the antispam database.
I don't have trackbacks enabled so I have no idea what the best defense against trackback spam is.
