IMHO it's often less confusing to _not_ have the wide design by default.
Maybe it should be just made more clear, that it can be switched?

Yeh, Simple / Advanced is a good idea in my opinion. Personally i use Simple mainly, and Advanced only when i need to, which i believe was the intended purpose of having them this way. So people don't mess up permissions.
Maybe a cookie to remember which layout you prefer, but imho simple should always be default.

Dunno how it happens, but the back office tends to remember a lot of little details about what I click on. This should be no different! It probably should get new names, but for sure it should remember that I always click on wide.

Ok. Changed it to "Simple" and "Advanced" and added localization support to it (it has not been translated before).
About saving it into UserSettings: I've added it for 1.9 for non-JS (changing layout reloads the page) and added a TODO for switching by Javascript (which would require an AJAX request and we have not settled on a framework yet).
