I'm not sure there's a manual that covers it, but it's in every skin you can get from the skins repository. This particular version comes from .9.1's custom skin and assumes you have a folder called "img" which you always do upon installation, and that it has an image called "b2evolution_log_80.gif" which you always do upon installation. AFAIK there are a handful of b2evo buttons in that folder, and no one minds if you use plain old text. It's the idea of a linkback as part of your _main.php that makes people (like me) happy. For the record it came from the end (or bottom) of the sidebar section of _main.php file. Some skins have it at the bottom in a footer type of thing.
To do it with text is pretty easy.
